Before a caregiver starts an ID-card application
A caregiver's Registry relationship and caregiver ID card are separate steps. OMMU says the patient's qualified physician is the only person who can add a caregiver to the patient's Registry profile. Check with the patient and physician that the relationship appears before starting the card application.
Since July 27, 2026, OMMU says a caregiver cannot complete an initial or renewal ID-card application unless connected in the Registry to a valid, active patient.
